Registering Online During Covid-19
If you have recently moved into the area please check if you live within the practice boundary before completing the registration form. If you do not live within the practice boundary, the practice will not be able to accept your registration application.
If you are not moving outside the boundary of your current practice you are advised to remain temporarily with your current practice to ensure that you can continue to receive care from a health care professional who has immediate access to your full medical records.
You are currently not required to provide evidence of identification when registering, but it would be helpful to us that you do if possible. A scanned copy or photo of one of the following is acceptable, Passport, Birth Certificate, Biometric Residence Permit, Travel Document, HC2 Certificate, ARC Card or Utility Bill.
We would also appreciate where possible to include your Next Of Kin details with you registration.
Registration may take up to 72 hours and we will accept applications via email during these unprecedented times to:
